Why a small joint causes a large problem
Arthritis of the big toe joint — hallux rigidus — is one of the most common forms of arthritis in the foot. The joint has to bend roughly to a right angle every time you push off, so once it stiffens, walking itself changes.
People adapt by rolling off the outside of the foot. That adaptation is what usually brings them in: pain under the ball of the foot, a sore forefoot after a day of standing, and new complaints in the midfoot, ankle or knee.

Primary treatment

Surgical options for an arthritic big toe range from removing bone spurs to fusing the joint. Fusion reliably reduces pain but ends motion at that joint permanently, and the load it used to take has to go somewhere else.
